Job Description
The Ambulatory Case Manager provides intensive Case management in a process that includes assessment, planning, implementation,coordination, monitoring and evaluation of the member's needs.Requirements:* CA Registered Nurse with a AS, BS or higher degree*CCM certificate preferred*Minimum 5 years clinical experience( managed care preferred)*Bilingual skills in with English/Spanish or English/Vietnamese highly desired*Knowledge of case management and utilization management guidelines and regulations, legal and ethical issues pertaining to CM, effective charting practices and guidelines.
