As a Certified Nursing Assistant, you will:
*Provide personal care and selected housekeeping and therapy services to ill, disabled, or injured patients within their home
*Assist with basic activities and daily living needs of patients
*Performs work assignments in accordance with organization's established policies, procedures, and protocols as directed by your supervisor.
*Meet with your PCC, on a regularly scheduled basis to assist in identifying areas for improvement.
*Identifies patients appropriately before administering treatments, serving meals, etc., as necessary.
*Record all entries on flow sheets, notes, charts, etc., in an informative and descriptive manner.
*Communicates changes in the patient's condition to the Team Leader/PCC as soon as practical. Report all accidents and incidents you observe on the day that they occur to Team Leader/PCC. Complete appropriate incident report.
*Report all complaints and grievances made by the patient/caregiver to Team Leaders and PCC.

Qualifications
*Current CNA license
*Six months or more experience in home health or related field preferred
*Ability to provide partial or complete living assistance to patients without assistance from other healthcare workers
*Effective communication and organization skills
