Multiple positions.

Work as a Community Based Flexible Support (CBFS) Outreach Worker on the Collaborative Action for Recovery (CAR)/Assertive Community Treatment (ACT) team to provide community-based rehabilitation/case management, support and supervision to CBFS clients with psychiatric and co-occurring disorders; provide consultation and training to clients and other team members on mental illness, psychiatric treatment, symptom/illness/wellness management, evidence-based clinical treatments, trauma-informed care, engagement strategies, crisis intervention, relapse prevention and risk management; conduct initial intake assessment as well as risk assessments, critical needs assessment and comprehensive psychosocial assessments of CBFS clients; develop clinical formulation and prioritize service needs; develop Individual Action Plans (IAPs) using rehabilitation values, principles and techniques and develop clinical formulation to guide development of IAPs; provide direct clinical services to CBFS clients as needed, including individual and group psychotherapy using evidence-based practices, psycho-education and other therapies such as cognitive-behavioral therapy, dialectical behavior therapy, trauma-informed interventions, motivational interviewing, relapse prevention and risk management; provide clinical coordination and liaison services for CBFS clients with community and hospital resources; attend or lead CAR/ACT team meetings and IAP meetings; participate in regular quality improvement and utilization management activities consistent with CBFS policy practice; and provide medication administration to clients as needed. Must have Master's degree in Social Work, Psychology, Counseling, Psychiatric Rehabilitation, Rehabilitation Counseling or related field and 1 year of experience in the job offered or 1 year of experience providing psychiatric rehabilitation interventions to, composing treatment plans for and performing psycho-social assessments of people with psychiatric disabilities and/or co-occurring disorders in community psychiatric settings. Must have LMHC or LICSW licensure or eligibility based on completion of a Master’s level degree that leads to licensure. Must have access to an operational and insured vehicle and be willing to use it to transport clients (will spend up to 60% of time in the community transporting clients to appointments and meeting with clients).
