Job Summary: Using independent nursing judgment, will assess, plan, facilitate, and integrate care for patients from preadmission to post discharge (Continuum of Care). Serves as patient/family advocate. This integrated care is consistent with the Case Management Department goals and hospital-wide clinical and financial initiatives. The Case Manager will identify system variances that prevent patients and clinicians from attaining achievable outcomes within a reasonable time frame. The Case Manager will also provide a more efficient, cost effective approach to administering patient care services. This will be achieved by ongoing proactive plans and interventions to positively impact quality and safety outcomes, length of stay, and reimbursement for both inpatient and outpatient populations.

Qualifications: Bachelor s degree in nursing from a recognized college or university, Master s preferred. Certification in specialty (Clinical, Case Management, Quality, and/or Utilization Management) preferred. Current RN license to practice in NC (NC license or multi-state (compact) license). Three to five years nursing experience. Experience in case management, home health, quality and/or utilization management preferred. Thorough knowledge of general nursing principles, methods, techniques. Exemplary verbal and written communication, critical thinking, interpersonal , group, leadership, and negotiation skills. Ability to communicate well with patient, family, staff, and physician to promote positive outcomes. Proficient in computer skills and MS Office software. Relief
