Case Manager: Provide services on behalf of consumers with severe and persistent mental illness. Experience in the field of Addications and Substance Abuse is strongly preferred. Willingness and ability to work with individuals of varying socioeconomic, ethnic, and educational backgrounds. Familiarity and working knowledge of community resources.
P Major skills needed: Complete individualized intake assessments; maintain accurate case records; maintain ongoing communications with medical and social service professionals and criminal justice personnel; coordinate and refer to support services, including housing, financial, legal, food, clothing, psychiatric and medical treatment; testify and/or accompany to court and legal appointments.
