SUMMARY: The Head of Social Services is accountable to the Vice President of Patient Care to ensure the effective and efficient provision of discharge planning, transition of care management, patient support and other services. Responsible for planning, implementing, coordinating and evaluating the operations of the Social Services department. Oversees the supervision and allocation of resources to ensure delivery of professional services which address the physical, psycho-social, spiritual, cultural, development and age-appropriate needs for each individual patient/family. This is accomplished through the incorporation of the Liberty Hospital mission, vision and values with the core values of the Clinical Practice Model.
Required Education/Training: Master of Social Work (MSW)Required Experience: Minimum five (5) years healthcare social work experience Full Time
