Social Worker - Case Management Social Services - Per Diem, as needed -02270-3126 Primary Location: United States-Nevada-Las Vegas Job: Case Management Schedule: PRN/Per Diem Description POSITION SUMMARY (includes, but is not limited to): The Social Worker is to provide patients, age s neonate through geriatric, with information and resources, which will allow them to function at an optimum level upon discharge from the hospital. He/she shall possess the ability to assess data reflective of the patient s status and interpret the appropriate information needed to identify each patient s requirements relative to his/her age-specific needs. This includes knowledge of the adoption process in the state of Nevada to facilitate the process between birth parents, adoptive parents and adoption agencies. Maternal/child health knowledge concerning education of teen parents, assessment of Level I and II nursery infants and families, providing emotional support, referral to community agencies as appropriate and coordinating with child protective services when appropriate. He/she will also have knowledge of financial entities to assist patients with obtaining assistance through social security, Medicaid and county. Must have knowledge of the Legal 2000 process and mental health resources. Qualifications Education: -BSW or MSW from an accredited school or university. License: -Licensed to practice as a Social Worker in the State of Nevada. Certification: -Basic Life Support (BLS) Experience: -Minimum of 2 years' experience in a medical setting with good knowledge of medical terminology and disease process. May consider new graduate. Special Skills: -Must have strong communication skills both verbal and written, computer skills and knowledge of community resources.
