Celebrate Caring at Salem Care and Rehabilitation
Center, a SunBridge Healthcare affiliate. We're seeking
a full time Social Worker at our 112-bed skilled nursing
center located in Salem, WV.
As our Social Worker, Primary responsibility is to
and/or assist with and participate in the planning,
organizing, developing, of the Social Services
Department in accordance with current federal, state,
and local standards, guidelines and regulations; Company
established policies and procedures. The focus is to
assist each resident and family in adjusting to
placement, illness, and plan of care to attain the
highest practicable level of functioning.
Requirements:
* Bachelor degree in social work, BSW, human services,
or related field.
* One-year experience working with a geriatric
population or in a long-term care setting preferred.
* Social Work license in accordance with state and local
standards.
* Ability to make independent decisions, to follow
instructions, and to accept suggestions for performance
improvement.
* Strong interpersonal skills in order to work with and
address issues with residents, family members, visitors,
government agencies/personnel and the general public.
* Ability to work harmoniously with and supervise other
personnel and develop/maintaingood personnel relations
and employee morale.
* Ability to manage multiple tasks and projects.
Ability to participate in and lead Interdisciplinary
Teams.
* Good communication and computers skills.
