    3. Click on the Birth-date field, and type your birth date as a two-digit month, two-digit day, and four-digit year.
    4. Select the Ethnicity radio button that describes you best.

    5. Select the Race check box that describes you best.
      Or
       Leave the None Selected check box as your choice.
    6. Select the Disability Status radio button that describes you best.
      Or
       Leave the None Selected radio button as your choice.
    7. Scroll through and read the Notice of Nondiscrimination and Right to File a Complaint statement, and click the CONTINUE button. The User Name/E-mail and Password screen appears.
