Job Description

Objective
Review and process medical records for completion. Essential Functions:

- Review each charge ticket/medical record for completeness:

- Patient Name
- Date of Service
- Start and Stop Time
- Referring Physician
- Diagnosis Information
- Procedure Code(s)
- Modifiers or Ancillary Charges
- Follow-up on missing information and document accordingly.
- Code the charge ticket for the following:

- ICD-9CM code to the highest level of specificity.
- CPT code (Review physician-reported codes and select the one with the highest base units
- Location
- Financial/insurance class and statement flag.
- Insurance company(s) in COB rank order.
- Case mix type.
- Appropriate modifiers and/or ancillary charges based upon patient s insurance carrier s requirements.
- Follow OB grid for OB epidurals.
- Compare the charge ticket with the anesthesia record to insure that no charges are missed and that the charges reported on the charge ticket are documented in the anesthesia record (when available).
- Create a charge batch ticket.
- Forward coded charge batch for demographic entry.
- Assist with patient phone calls
- Other duties as assigned. Education Requirements:

High school degree or equivalent; coder certification preferred Experience Requirements:

0-3 years providing coding support services
