Registered Nurse / RN Job Description:
The RN Salaried Case Manager, who is licensed professional, coordinates care between patients, families, and health care personnel to ensure efficient and prompt health care services. Providing direct and indirect client care services under a physician's plan of care and in accordance with policies and procedures of Home Health Care by Black Stone, Ohio's professional standards, and regulatory entities.
LPN's and HHA's
Education and Experience:
* Registered Nurse with an Ohio license
* Has at least one year of experience as a professional nurse, or
* Prefer a BS in Nursing from an accredited program by the National League of Nursing or
* Has worked as a registered nurse at least one year within the last 3 years or completion of a refresher course in the last year.
* CPR certification required or obtained within 90 days of hire date
* ICCM within 1 year of employment
* Access to utilization of public or private transportation system.
* Ability to lift, push, pull light to moderate loads
* Read, write, and communicate in English
* Basic computer skills
* Knowledgeable of nursing and medical practices, procedures, regulations and guidelines that pertain to home health care.
* Ability to lift 30 pounds, move intermittently throughout the work day, inclusive of sitting, standing and bending, moving up and down flights of stairs, squating, pushing, pulling, or moving objects and clients.
