Clinical: Conducts comprehensive assessments of patients health status by taking health histories and performing advanced physical examinations in specialty area with heart failure and other cardiopulmonary diseases. Provides direct clinical care to assigned patients as needed on heart failure/pulmonary unit and in our inpatient observation unit.. Orders and interprets laboratory and diagnostic tests to establish diagnoses and develop a comprehensive treatment plan. Prescribes medications, therapies, interventions and procedures according to clinical privileges. Documents in the medical record all care provided and/or procedures performed for which the Advanced Practice Nurse is responsible. Collaborates with all members of the multidisciplinary team to develop and direct an individualized and comprehensive treatment plan. Coordinates the discharge planning process with patients, families and all members of the multidisciplinary team. Consults with other specialties and disciplines as appropriate to meet all health care needs of patients. Acts as Consultant to other disciplines in specific areas of expertise.
Leadership/Organization: Advocates for patients, families, nursing staff and other members of the healthcare community. Maintains advanced certification and continuing education as required by the Illinois Department of Professional Regulation and Advanced Nursing Practice Act. Assumes a leadership role in professional activities and organizations. Serves as a patient advocate and role models clinical practice that reflects "Patients First" philosophy. Assists in the development of policies and procedures in clinical area of expertise to establish standards for nursing practice. Participates in department and hospital committees and task forces to facilitate changes and develop processes to improve patient care utilizing evidenced based practice guidelines. Consults with nursing and hospital leadership (management) in the identification, evaluation and problem solving of clinical system and process issues. Collaborates with NM leadership teams to develop, implement and evaluate strategic plans for quality improvement initiatives. Collaborates with physicians to lead changes in clinical practice. Collaborates with leaders in academic, political and financial institutions to promote health care reform. Serves as a consultant to the department of nursing in developing strategies for advancing nursing practice and leadership opportunities for nursing staff and APNs.
Education: Develops and provides comprehensive teaching strategies for patients and their families. Consults with patients and families regarding health care needs, health promotion, health management and disease prevention strategies. Develops and participates in programs to meet educational needs of nursing staff and other healthcare professionals related to clinical area of expertise. Serves as a role model and preceptor for undergraduate and graduate nursing students. Provides formal and informal education to both internal and external groups pertinent to area of expertise. Provides formal and informal educational programs to nursing staff graduate students and other healthcare professionals pertinent to area of expertise. Collaborates with academic institutions to provide and promote clinical learning experiences and educational programs for APNs, PAs, physicians in training and all health care professionals.
Research: Identifies relevant areas of clinical research and assists with developing and coordination of research protocols. Coordinates and conducts independent and collaborative research within area of expertise. Collaborates with the multidisciplinary teams to conduct research and participate in clinical trials. Consults with members of the research team to assist in interpreting research findings and developing strategies for implementation of results into evidenced based clinical practice guidelines. * If an APN, current license as a Registered Nurse in the State of Illinois, Current license as an Advanced Practice Nurse or Physician Assistant in the State of Illinois.
* Certification as an Advanced Practice Nurse by a national certifying body as approved by the Illinois Department of Regulation APN Board. Or Masters Degree from an Accredited Physician Assistant Program.
* Written collaborative agreement with a collaborating physician and once licensed may apply for a mid-level practitioner controlled substance license and DEA number.
* Privileged and credentialed through the NMH Medical Staff Office (MSO).
* Cardio-Pulmonary Resuscitation recognition (healthcare provider level).
* Masters degree from an accredited school/college of nursing.
Two years clinical experience in related area of specialty preferred but will consider new graduates of accredited PA or Advanced Practice Nursing Program.
