ARNP - Nursing Home Services (Per diem)
Our Nursing Home Services Division is passionate and committed to maintaining high quality care for older adults. We have established partnerships with a select number of facilities in the Puget Sound Region to provide a continuum of care that achieves excellence in quality measures, patient/family satisfaction, along with clinical and utilization outcomes (including re-hospitalization). We provide care from Olympia to Everett, and Issaquah to Silverdale
Group Health ARNPs provide medical management of patients during skilled/Medicare Part A or subacute stays, as well as long term care/residential stays in skilled nursing facilities in a team based model.
Minimum: Masters Degree in Nursing from an accredited school of nursing
PREFERRED: Masters Degree with gerontology or acute care specialization
MINIMUM: Advanced geriatric clinical expertise in acute and chronic health problems. Strong problem solving/decision making skills. One-year of clinical experience as a nurse practitioner managing acute and chronic complex medical issues and coordinating care across the continuum.
MINIMUM: One-year of geriatric clinical experience as a nurse practitioner managing acute and chronic complex medical issues and coordinating care across the continuum.
PREFERRED: One or more years of clinical practice in skilled or long term care settings.
MINIMUM: At time of hire Washington State RN and ARNP license, national certification. Prescriptive authority, including Schedule II-IV medications. BLS/CPR certification.
PREFERRED: Gerontology or acute care specialization
