The Edward J. Healey Rehabilitation and Nursing Center is a 198-bed comprehensive skilled nursing and rehabilitation facility. Serving the needs of the community since 1917, the Edward J. Healey Rehabilitation and Nursing Center provides long-term and short-term care in a comfortable setting.

General Statement of Job
This position gathers all information required by facility's admission criteria in order to consider a resident for admission. This position receives referrals, interacts with community, and coordinates all admissions into Edward J. Healey Rehabilitation and Nursing Center. This position is responsible for performing such in an efficient and expeditious manner.
Specific Duties and Responsibilities
Essential Functions:
Gathers sufficient pre-admission data to determine resident level of care and the ability of the facility to properly meet the needs of the resident.
Interviews potential residents and residents' representatives to collect accurate data.
Completes pre-admission and admission forms accurately and timely.
Explains admission agreement and all attachments and obtain signatures.
Explains facility philosophy, policies and procedures to potential and new residents and residents' representatives.
Interacts with discharge planners in an effective, efficient manner. Request and analyze information to determine if facility can meet the needs of the resident; advises discharge planners of final outcome.
Provides tours of the facility.
Receives inquiries and provides information about the facility and admission process.
Responsible for admitting Residents into the facility in a smooth, welcoming and expeditious manner.
Prepares resident identification bracelet, face sheet and takes photographs for identification purposes.
Requests information about advance directives from resident or representative.
Reviews Residents' referral charts for completion. Calls referring health care facilities, outside agencies, and private individuals to request additional medical, social, and financial information needed to complete processing Residents' applications.
Codes all Resident diagnoses upon admission and inputs in ADL computer program.
Accurately inputs all resident demographic information, including insurance and billing information in the ADL computer program. Prints the admission records, copies and distributes.
Notifies appropriate departments when a resident is admitted; assists with notification when a resident is discharged, transferred or moved from one room to another.
Refers facility and resident problems or concerns to appropriate individuals.
Provides group and one-on-one presentations to health care providers and agencies, updating them on the admissions criteria.
Participates in marketing Edward J. Healey Rehabilitation & Nursing Home and maintaining acceptable census level.
Makes visits to home, hospital or other facility and performs nursing assessment to ensure facility can meet needs of prospective resident. Review medical history, medications, social and psychosocial needs.
Contacts case managers when resident is hospitalized to monitor condition.
Emergency duty may be required of the incumbent that includes working in Red Cross shelters or to perform other emergency duties including, but not limited to, responses to threats or disasters, man-made or natural.
