ADOLESCENT IOP GROUP FACILITATOR
Centennial Peaks Hospital is a 72 bed facility located
in beautiful Louisville, CO. We are dedicated to
providing a full range of mental health and chemical
dependency services to the state of Colorado. We strive
to enhance the mental health, safety and well-being of
our patients, their families, and the community at large
by providing ethical, compassionate, medically and
psychologically integrated inpatient and intensive
outpatient treatment for psychiatric and addiction
disorders.
JOB SUMMARY:
The Adolescent IOP (Intensive Outpatient Program) Group
Facilitator provides quality direct and indirect patient
care to individuals in the mental health and chemical
dependency outpatient program, Maintains accurate
program and patient documentation, including medical
records, develops and implements a plan of treatment, in
conjunction with members of multi-disciplinary team,
conducts groups, communicates with referral sources and
other appropriate parties regarding patients' progress,
and coordinates patients' discharge planning and
implementation.

Requirements:
JOB SPECIFICATIONS:
To perform this job successfully, an individual must be
able to demonstrate competency in the criteria listed in
this job description. The specifications listed below
are representative of the knowledge, skill, and ability
to meet the minimum requirements for this position.
Staff must demonstrate flexibility for reassignment to
provide care to varied populations including adults and
dually diagnosed patients in an inpatient and partial
hospital setting.
Education: Master's degree in Social Work,
Clinical/Counseling Psychology or Social Services
related field, as well as additional CACII/CACIII
training in substance abuse treatment is preferred,
licensure is preferred. License eligible degree is
required.
Experience: Prior experience working with a psychiatric
mental health population, preferably in an inpatient
setting. Must have experience with Adolescents and
family dynamic's
Licensure/Certification: IOP Facilitator: CACII/CACIII
preferred, LCSW, LPC, LMFT preferred.
