JOB DESCRIPTIONAdvanced Practice Nurse:
-Coordinates and completes the evaluation phase for patients referred for surgery.
-Uses care plan and/or clinical path in providing patient care. Delegates as appropriate based on patient needs.
-Works with patients and their families to provide education and establish plan of care, starting from initial evaluation and following the continuum of care including pre-procedure, post procedure, and continued follow-up post surgery.
-Coordinates all aspects of discharge planning.
-Participates in outpatient management in support of optimal patient outcomes.
-Serves as a liaison among physicians, staff, families and payers to ensure effective utilization of resources and coordination of care.
-Communicates effectively to patients, families and other team members in relation to health care maintenance.
-Promotes, has knowledge of, and utilizes appropriate patient care protocol relating to the physical, psychosocial and developmental needs of transplant, heart failure, LVAD, Valve etc. patients.
-Participates in daily Cardiothoracic rounds with the multidisciplinary team.
-Contributes to creating a work climate that encourages positive staff morale, motivation and commitment for all who participate in the care of patients.
-Incorporates cost effective measures into practice to assure proper use of time, supplies and personnel.
-Shares in professional leadership responsibilities related to patient care at various levels by directing and teaching other employees.
-Provides support to peers by assisting as needed, being a resource person and working as team member.
-Participates in the department quality improvement program and strives to optimize patient outcomes through patient education and clinical expertise.
-Gathers and reports data as mandated to external databases such as STS etc. to ensure quality outcomes and reporting initiatives.
-Participates in related conferences and team meetings.
-Facilitates and participates in clinical research.
-Provides education to internal staff as well as external community resources.
-Using clinical pathways and/or plans of care incorporating standards of practice.
-Assesses own need for professional growth and development and seeks appropriate resources to meet needs.
-Attends continuing education programs and participates in program evaluation.
-Promotes the professional growth and development of patient care team members by participating in the recruitment, orientation and education of new staff.
-Accepts responsibility for professional and personal actions.
-Assumes responsibility for maintaining current clinical specialty knowledge base.
