For Nurse Practitioner:

* Master s Degree in Nursing with certification as an advanced practice nurse is required.

* Licensure, or eligibility for licensure, as a professional Registered Nurse and an Advance Practice Registered Nurse in West Virginia required

For Physician Assistant:

* Minimum of Bachelor s Degree as a Medical Physician Assistant is required. Masters level Education as Medical Physician Assistant preferred.

* Graduate of an American Medical Association (AMA) approved Physician Assistant (PA) training program required.

* Certification by the National Commission for Certification of Physician Assistants required.

* Must be licensed or eligible for licensure by the WV Board of Medicine.


The Stroke/Neuro-Radiology Advanced Practice Professional is an individual with clinical skills in the care of stroke/neurology patients and families. Completes comprehensive patient health evaluations in collaboration and consultation with the Neurologists, and the Neurosurgeons. Assesses and coordinates the educational and psychosocial needs of the patient and family. Reviews the plan of care with the clinical team, coordinates and participates in/directs the patient s care, coordinate continuity of care for pre/post procedural patients, and manage discharge planning in collaboration with Faculty and clinical team..

ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned.

1.0 Manages comprehensive and individualized care for stroke patients/neuro patients and their families in the inpatient phase of hospitalization.

1.1 Completes daily rounds and/or pre/post procedural evaluations.

1.2 Completes integrated assessments.

1.3 With the collaboration of Neurology/Neurosurgery, or by following established clinical Guidelines and Protocols, orders additional tests or consults.

1.4 Performs, interprets and communicates results of common diagnostic tests in the

clinic and hospital setting.

1.5 Consults and communicates with Neurology/Neurosurgery, clinic and hospital staff regarding abnormal findings in the evaluations of patients.

1.6 Performs history and physical examinations and documents findings in progress


1.7 Facilitates patient discharge be coordinating appropriate services and tests prior to

scheduled outpatient follow-up appointments in the clinic.

1.8 Performs necessary procedures following established clinical guidelines and Protocols and scope of practice

2.0 Manages comprehensive and individualized care through the inpatient and immediate post-discharge phase of the patient s hospitalization experience.

2.1 Develops a comprehensive individualized plan of care for patients, which includes teaching, discharge planning and necessary referral to other members of the health care team, with specific emphasis on anticoagulation therapy management.

2.2 Demonstrates expertise in caring for patients across the age continuum.

2.3 Participates in physician rounds, as applicable

2.4 Identifies unstable or complex patient care situations and facilitates appropriate intervention in the clinic (as applicable) and hospital setting.

2.5 Facilitates patient discharge by coordinating appropriate services and tests prior to scheduled outpatient follow up appointments.

2.6 Evaluates patients in the outpatient setting. Acts as a coordinator for follow up procedures and tests in the outpatient setting (as applicable).

2.7 While in the clinic setting, assists in the collection of outcome measures (as applicable).

2.8 Demonstrates critical thinking and diagnostic reasoning skills in clinical decision making by the identification, evaluation and management of the individual needs of patients/families.

2.9 Demonstrates the ability to utilize basic equipment in a safe manner.

2.10 Recognizes emergency situations and initiates effective emergency care.

2.11 Bases decisions made/actions taken on relevant scientific principles, established standards of care and/or research findings.

3.0 Utilizes teaching/learning theory and appropriate teaching strategies in the provision of

education to nursing staff, hospital staff, patients and families, and the community.

3.1 Promotes an environment that facilitates learning by responding to appropriate

Patient/family preferences, priorities and readiness to learn.

3.2 Evaluates the effectiveness of patient/family teaching and collaborates with the nursing and clinical staff in the completion of education plan.

3.3 Updates own knowledge base throughout the year.

3.4 Completes all identified clinical competencies.

3.5 Serves as a resource person for staff caring for neurology/stroke/population.

3.6 Provides educational programs to professional and paraprofessional personnel in

area of expertise.

3.7 Mentors nursing personnel in all levels of clinical practice.

3.8 Participates in community programs, distribution of literature, as appropriate in service area..

4.0 Communicates effectively with patients/families, physicians and ancillary services at all levels to assure coordinated efficient patient care.

4.1 Documents the plan of care, interventions and evaluations in a clear and concise

manner in the patient s medical record following approved hospital/UHA policies for documentation and compliance standards.

5.0 Collaborates with other leaders and staff to effectively achieve needed change.

5.1 Provides leadership in the development and implementation of changes in practice that positively impact patient outcomes.

5.2 Works collaboratively with medical and nursing staff to facilitate creative problem solving and provide professional support.

5.3 Participates and initiates inter-disciplinary approaches to patient care, program development and education. Serves as a professional role model.

5.4 Promotes and practices innovation of the expanded role in the delivery of care to patients and families throughout the inpatient hospitalization or clinic setting.

5.5 Participates and provides leadership for committees and task forces..

5.6 Documents hours and submits billing reports, as appropriate.

6.0 Participates in the WVU Stroke Program Performance Improvement Plan.

6.1 Applies information from Performance Improvement activities, changing practice as necessary, to improve care provided to patients.

6.2 Reviews and interprets stroke data from sources such as the stroke registry, radiology data clinical and financial reports, length of stay, and cost per case reports and recommends/implements changes as necessary.

6.3 Participates in clinical trials and research studies with the Principal Investigator.

6.4 Assists Neurologists/Neurosurgeons and Administrative Director in the collection of outcome measures.

7.0 Other duties/projects as assigned.
