Assembly & Analysis Clerk
40 hours a week 8:30 a.m. - 5 p.m.


Prompt receipt and entry of the record into the system at discharge from inpatient status or conclusion of ambulatory care.

Verification check (reviewing each document for presence and correctness of patient identification and proper unit number assignment).
�
Processing of documents according to standard chart order.
�
Initial review for completeness. Determines that documentation is appropriate to the particular episode of care.
�
Determines proper authentication of all entries.
�
Performs a detailed analysis of the record to determine chart deficiencies.
�
Enters all deficiencies into the computer system and attaches a deficiency list to each patient chart as necessary.
�
Performs monitoring and notification process for completeness.
�
Prepares reports concerning compliance/noncompliance with record completeness standards for medical staff or administrative action.
�
Fosters respect for patient privacy by maintaining confidentiality in all phases of the work.
�
Understands the impact and consequence of maintaining the Incomplete Record system by applying procedures and making situational decisions based on those procedures with regard to reinstating suspended medical staff privileges.

Requirements:
High School diploma or equivalent. One to two years experience in an acute care Health Information Management Department required. Knowledge of medical record content, sequence, and terminal digit filing. Completion of medical terminology course preferred. Experience with a hospital computer system.
