Demonstrates responsibility and accountability in the performance of patient monitoring responsibilities in accordance with Department/Unit Standards and patient's orders
Evaluates behavioral observation/changes in patient status - coordinates and implements interventions with treatment team.
Continuously prioritizes patient and milieu needs based on assessment/evaluation; again implements with treatment team.
Demonstrates an ability to exercise independent judgment in time of need and emergency situations
Performs comprehensive psychosocial assessments upon patient admission
Demonstrates knowledge of specific behavioral health disorders and treatment modalities for the specific patient population of the Department/Unit
Collaborates with other members of the multi-disciplinary team/family/SO in implementation of the Plan of Care
Documents patient's response to the Plan of Care, communicates to the health care team and implements changes in collaboration with the team.
Makes appropriate and timely referrals as patient need(s) are identified
Reports circumstances which might adversely affect safe and therapeutic care to the appropriate person in a timely manner
Identifies and documents patient/family/SO education in collaboration with other disciplines and implements initial and ongoing education relevant to those needs
Job Qualifications include:
* Post Grad degree
* LCSW or LMHC required
* 1-3 years of experience in a hospital setting preferred
* Able to conduct individual, group, family and marital therapy
