You don t have to be in direct patient care to work at Northside. The Care Coordination Department allows you to work in a business setting, while still being part of an organization that gives compassionate care for people. We provide our patients and clinical staff with support to determine the appropriate setting of care and appropriateness of hospitalization, as well as to assess post hospital needs. Through discharge planning and utilization review, the Care Coordination Department provides the necessary interventions in order to assure effective and efficient clinical resource management at our campuses.

This position will work in our Emergency Department at Northside Atlanta. The Care Coordinator will work on a 4, 10 hour schedule that will rotate through the weekends.

Required: 1. Graduate of an accredited Registered Nursing School (RN) with a current GARN license 2. One (1) year previous experience in case management, utilization review, or social services. 3. Ability to interact with patients, families, physicians and hospital staff. 4. Excellent oral and written communication skills. 5. Ability to type accurately at an approximate rate of 40-45 wpm.

Preferred: 1. BSN 2 Three (3) years experience in case management, social services or related field. Prior Emergency Department Care Coordination experience.
