Responsible for the efficient, timely coordination of health care services provided to a defined patient population. Responsibilities include monitoring and facilitating cost-effective care, identifying opportunities for improvement and coordinating and/or initiating interventions when opportunities are identified, facilitating improvement in organizational performance, assuming a leadership/educational role in the interdisciplinary health care team, and advocating for the patient/family at the service-delivery level. Graduate of an accredited BSN program requiredCurrent RN licensure in Virginia or eligible3-5 years clinical experience in an acute care setting required1 year of UM/Case Management experience preferred. Ability to interact with all levels of the organization, patients, families and outside agencies
