Description

Creating and assisting with the expansion of business opportunities and in the development and implementation of educational activities that increase patient volumes to agency through relationship with host facility or facilities.

Will use independent judgment to make recommendations for decisions regarding patient's acceptance and admission.

Will coordinate services between referral source and appropriate service line representative (Home Health, Hospice, Pediatrics, and Primary Homecare and other service lines as appropriate to location).

Will provide education and information to patients, patients' families, hospitals, nursing homes, physicians and other community entities.

Will obtain all required consents for agency services that are being requested through meeting with and offering solutions to prospective patients and families.

Will build effective relationships with referring staff at hospitals, nursing homes and other care facilities. Must be able to work a flexible schedule and travel.

Requirements

Will provide \\"first response service\\" to all referrals that will guarantee immediate patient and family accessibility to evaluation and admission resources. Visit a minimum of 4-5 patients / families per day, completing visits in an average of 45 minutes.

Assess patient / family needs, identify unarticulated needs, offer CHRISTUS Care at Home solutions as appropriate and complete various registration forms.

Uses independent judgment to make recommendations for decisions regarding patients. If required, will consult with clinical staff. Refer patient and/or patient's family to appropriate resources and instruct patients and families on the scope of services.

Identifies and acts on problem resolution with customers.

Develops, creates and conducts community education in-services with in the scope of CHRISTUS Care at Home abilities and licensure will allow.

Conducts referral development contacts when registration visits are not scheduled and when registering patients at facilities.

Prepares a weekly report of referrals/patients work-ups, actions, and activities.

Performs other job duties as assigned.
