SERVICE AREA OVERVIEW
You don t have to be in direct patient care to work at Northside. The Care Coordination Department allows you to work in a business setting, while still being part of an organization that gives compassionate care for people. We provide our patients and clinical staff with support to determine the appropriate setting of care and appropriateness of hospitalization, as well as to assess post hospital needs. Through discharge planning and utilization review, the Care Coordination Department provides the necessary interventions in order to assure effective and efficient clinical resource management at our campuses.

POSITION DESCRIPTION
The Care Coordinator SW will be responsible for working with neonate, adolescent, adult, and geriatric patients, families, medical staff and community agencies to determine the appropriate setting of care and appropriateness of hospitalization, as well as to assess post hospital care needs. Provides the necessary interventions in order to assure effective and efficient clinical resource management. Provides intervention regarding adjustment to hospitalization, crisis intervention and psychosocial assessment. Will be actively involved in Discharge Planning and Utilization Review in a progressive acute care setting.

POSITION REQUIREMENTS
Masters of Social Work (MSW) OR B.S. degree in related field with strong clinical background. One (1) year previous experience in case management, utilization review, or social services. Ability to interact with patients, families, physicians and hospital staff. Excellent oral and written communication skills. Ability to type accurately at an approximate rate of 40 wpm.

Preferred: Masters of Social Work, Three years of case management, utilization review and/or discharge planning, experience in an acute healtchare setting.

WORK HOURS: 8a-4:30p

WEEKEND REQUIREMENTS: Yes
