Department: Care Management

Serves as a patient advocate/liaison, overseeing the continuum of care for the patient from hospital admission to discharge. The RN care manager will take a proactive role by early intervention and collaborative practice with staff nurses, physicians, ancillary services, payer sources, and outside agencies assuring appropriate use of resources while focusing on positive outcomes. Works with payers and gatekeepers to assure necessary precertifications / authorizations are obtained. Reviews patients level of care daily, using Interqual criteria, to assure patient is being treated in the most appropriate level of care; documents these reviews daily in the Midas Plus system. Must be able to demonstrate the knowledge and skills necessary to provide care and service appropriate to the age of the patients served in his/her area.
Minimum Education/Training:Bachelors Degree in Nursing or other health care related field.Two years of Case Management/Discharge Planning experience.
Registration/Certification or License Required: RN licensure in the State of Michigan.Current BLS for the Healthcare Provider Certification (or completed within 60 days of hire).
Preferred Education: Formal training in Case Management and formal Criteria training.Accredited Care Manager.Certified Case Manager.
Preferred Experience: Five years of Case Management experience in a health care field. Experience with 3rd party reimbursement.
Management Experience (If applicable):
Physical Requirements: Frequent sitting, standing, walking.Lift 10 lbs. box overhead. Lift and carry 15 lbs. Push/pull 20 lbs. cart.
Other Requirements: Effective communication skills.Ability to work well under stress within a changing environment.Crisis management and conflict resolution skills. PTB Varied
