The Care Navigator is responsible for care coordination for members who require a comprehensive approach to immediate and/or ongoing care of their complicated and/or catastrophic medical illness. The Care Navigator documents interactions with and on behalf of the member throughout service delivery including, but not limited to: care plans, progress notes, correspondences, and authorizations.

Key Responsibilities:
Identify members who had a recent transition of care and assist with follow up and discharge needs

Monitor members identified through the High Utilization of ER Services Reports

Act as a liaison between Health Choice and additional services/programs, including, but not limited to: Medical Home, Care Plus Home Based Program, End of Life Care Program, Medication Management Program

Accept other job duties as indentified in the Transition of Care Program
Professional Competencies (knowledge, skills, and abilities):

Knowledge:
Knowledge of Medicaid guidelines and regulations preferred
Experience in Medicare/Medicaid highly preferred
Knowledge of medical terminology

Skills:
Computer experience necessary
Effective time management skills
Effective interpersonal and communication skills
Strong attention to detail

Abilities:
Maintain a positive, respectful work environment
Think analytically and make independent decisions
Management of large workload
Work cooperatively, positively, and collaboratively in an interdisciplinary team
Manage multiple projects and prioritize work tasks to adhere to deadlines and identified time frames

Education:
High School Diploma or equivalent GED
Associate s degree (AS) or Bachelor s degree (BS) preferred

Experience:
At least two (2) years in a health care or related setting
HMO/Managed Care/ Medicare/Medicaid experience preferred

Certification and License:
A license in a clinical field is preferred (e.g., LPN, RN, etc.)
Medical Assistant Certification preferred
