p STYLE=MARGIN TOP: 0px; MARGIN BOTTOM: 0pxRehabCare has an outstanding opportunity for a Registered Nurse as a Case Manager within our sub acute unit in As a Case Manager with RehabCare, you will: Develop programs for specialty subacute / transitional care unit programs in designated hospitals or facilities through education to the community including healthcare providers and managed care organizations Conducts on site physical evaluations or collects clinical data of potential subacute candidates for admission to the specialty program Conduct Off site evaluations for potential transitional care candidates on a case by case basis Provide accurate family information to social worker
