It takes a passion for meeting the behavioral health
needs of our communities and a special experience that
meets the high expectations patients have come to expect
of the Fairfax Behavioral Health team. We are currently
expanding into Snohomish County with a new 34-bed
psychiatric hospital serving older adults on the campus
of Evergreen Health Monroe. You could join our newest
team in one of our three Case Manager positions.
The Case Manager provides the following social services
functions to assist patients in meeting their
psychosocial needs while enabling them, through
therapeutic skills, to achieve an optimal level of
emotional health: treatment planning facilitator,
psycho-social assessment, psycho-education, short term
supportive family and individual therapy, group therapy
and discharge planning, including liaison with community
resources. The Case Manager utilizes evidence based
practice in assisting clients achieve their optimal
level of mental and emotional health.
Job Requirements
Education: Master s degree from an accredited college or
university in Social Work preferred; mental health field
Preferred three (3) years, post-Master s experience in
a psychiatric health care setting.
Direct experience in individual and family group
therapies, crisis intervention, and treatment planning.
Strong clinical assessment skills required.
Additional Requirements: Strongly preferred for the
employee be licensed in the state of Washington as a
Social Worker, or related mental health field. Full
licensure must be achieved with two years of employment
and group supervision for licensure may also be
available. Employee must be available to work weekend
rotations. Employee will obtain CPR, first aid, Handle
with Care, and Verbal De-Escalation certification.
