POSITION SUMMARY: Provides overall coordination in the delivery of services and discharge planning for specified patient population. Performs concurrent analysis of patient records for purposes of utilization review and quality assurance. Promotes a cooperative and supportive relationship as liaison with patient, family, facility staff, physicians and funding representatives. Ensures continuity in the delivery of services across St. David s entities.
REPORTS TO: Director of Case Management
SHIFTS: Part Time (16 hrs) - 8 hours Saturday and 8 hours Sunday
POSITION QUALIFICATIONS (REQUIRED/PREFERRED):
EDUCATION: (Required) Registered Nurse with clinically focused work history.
EXPERIENCE: Minimum three years clinical experience within an acute health care setting preferred. Prefer previous recent work history in acute care case management role. Working knowledge of case management philosophy/process/role, needs assessment, principles of utilization review/quality assurance, discharge planning, and reimbursement structures (i.e. Medicare, Medicaid, HMO).
KNOWLEDGE, SKILLS AND ABILITIES: Ability to communicate effectively, read and comprehend complex clinical and financial data; organize, prioritize and manage time efficiently. Utilizes computer keyboard extensively to obtain and transmit patient financial, medical and psychosocial information of a confidential nature. Operates printer to obtain computer-generated information. Utilizes telephone and FAX machine. Must be able to communicate effectively with a wide range of hospital personnel, physicians, patients/family members and representatives of outside agencies. Ability to work with patients of adolescent, adult and geriatric age.
* Must be able to demonstrate understanding of HCA s and St. David s Patients First safety initiative by strict compliance to all safety protocols and procedures.
* Recognizes that variables such as age, race, nationality, culture, disease-causing vulnerabilities, socioeconomic factors affect the patient populations under their care as defined in the unit s provision of care statements. Uses interventions appropriate to these factors (e.g., interpreter, dietary preferences, social service consult, etc.)
LICENSE/CERTIFICATION: Registered Nurse Licensure required. Preferred: BSN
