RHD Passages - Grubb Rd. is a residential program for adults with serious mental illness. Residents participate in services that focus on building skills of community living. The goal of the program is to maximize the ability of each individual to live as
independently as possible. Grubb Rd. is seeking CNA ADL Counselors to provide direct care services to group home residents. The CNA ADL Counselor performs tasks aimed at supporting the individuals needs and desires as part of a team.
Essential Duties and Functions
Performs tasks in a manner that upholds the Values of RHD
Interacts with consumers in a manner that reflects compassion and understanding.
Interacts with peers, supervisors and the community in a manner that reflects a respectful, mature and professionaloutlook
Interacts with consumers in a manner that is not confrontational or argumentative
Interacts with consumers in a manner that is respectful of the consumer s ability to make his or her own decisions, while also managing the responsibility of the program
Performs tasks in support of the physical and mental well-being of the consumers
Provides consumers with supervision, support and assistance with all activities of daily living including but not limited to:
personal hygiene tasks such as toileting, bathing, showering, shaving, oral hygiene, nail care
Preparing and serving meals, assisting with feeding if necessary.
Community outings and appointments
Supports the ongoing health of consumers by assisting with self-administration of medication, administering enemas, douches, nonsterile dressings, ice packs, heat treatments, sitz and therapeutic baths
Maintains patient stability by checking vital signs and weight; testing urine; recording intake and output information
Observes skin, behavior, general condition for changes; reports changes to RN immediately;
Completes all documentation as requested
Facilitates psychoeducational groups
Performs tasks to assure that program is in compliance with the policies and regulations of RHD, LTCRP, DSAMH, AND CARF
Renews on time and provides documentation of all items required for ongoing employment
Provides services in a manner consistent with the Rights of Clients as outlined by Delaware Code, Section 1121, entitled Patient s Rights
Performs tasks in the support of the well-being of RHD Delaware Mental Health Group Homes
Spends time on shift actively engaging consumers in growth activities, skill building activities or other purposeful activities
Communicates with peers throughout shift with regards to needs of consumers and tasks to be completed on shift
Works effectivelyand cooperatively as part of a team
Completes tasks on shift in a self-motivated manner
Communicates with on call staff regarding urgent programmatic issues
Participates in maintaining the cleanliness and order of the physical property
Demonstrates problem solving skills
Displays a positive attitude toward the job and tasks of the job
Takes initiative to address items that need repair or attention at job site
Follows policies and protocols aimed at ensuring the safety, well-being and growth of consumers
Participates in supervision with the Resident Manager/Unit Director for the purpose of ongoing professional development
Maintains a valid driver s license
Other duties as assigned
Current Delaware CNA Certification
Previous experience working with adults with chronic mental illness
Current driver s license
