Job Description

Care CoordinatorPurpose
Provide youth with supervision and services in coordination with family, facility-based staff, the community, and the Child Welfare System with the goal of successful reunification of the youth.
Duties
* Provide home visitation and family support services to children and families referred from DHS Agency worker.
* Provide direct services to assigned clients. This requires the ability to: o Ensure safe, legal transportation for self and others. o Meet at times and places convenient and appropriate to client needs, and be accessible by phone. o Effectively assess child and family needs, behavior and progress. o provide services to assure the safety of family members o Assure Children who are safely maintained within their families receive appropriate services; o Assure Children who are in out-of-home placement and the goal is reunification with their family or placement with a relative receive appropriate services and o Assure Children for whom the goal is to achieve or maintain alternative permanent family connections, including placement in an adoptive or guardianship arrangement receive appropriate services. o and taking mediation o Assist in implementing transition plans for older youth moving toward adulthood
* Conducts client/family interviews to obtain information for case summaries and histories, prepares assessments, social histories, service plans, case records, referral summaries and financial information as directed.
* Maintains face-to-face, phone and written contacts with clients, families and providers regarding client well being, treatment progress and casework plans.
* Assesses client work/employment and living situations; provides information and advice to clients on available programs and services.
* Evaluates reports of child or dependent adult abuse; assesses strengths/needs of clients and recommends service interventions.
* Performs outreach activities gathering and evaluating information regarding clients or programs, developing an assistance or treatment program, and coordinating activities with relevant community agencies, as directed.
* Communicate with DHS worker
* Develop and implement an Eckerd Individual Family Service Plan based on the DHS Agency Safety Plan and/or Family Case Plan with each family participating in Safety Plan and Family Safety, Risk and Permanency Services.
* Use the North Carolina Family Assessment Survey-R to determine child and family needs
* Based upon the NCFAS and the Family Safety Plan or Agency Family Case Plan Provide any or all of the following services to or on behalf of children and families: o Safety checks and supervision activities o Activities to arrange for respite care and/or protective child care assistance o Activities to monitor and ensure that a parent is keeping their mental health or substance abuse treatment appointments and taking required medication o Activities to inspect and monitor the safety of the home environment o Family Functional Assessment o Visitation planning and supervision of visitation o Crisis intervention responses o Family functioning interventions o Family reunification services and activities o Concurrent and permanency planning service o Coordinate and facilitate client referrals to appropriate community resources o Household management assistance and instruction o Transportation assistance o Activities to secure concrete supports o Individualized case-specific services
* Use specified Eckerd Family Engagement model and parenting curriculum with families in the program based on needs identified.
* Teach child development and parenting skills.
