The Chronic Kidney Disease Educator and Nutrition Specialist will lead the team in creating, developing and maintaining excellence in outpatient chronic kidney disease education. This individual will educate adults and their families in the outpatient nephrology clinic in both individual and group settings. Will work collaboratively within a multidisciplinary Chronic Kidney Disease team and utilize advanced clinical and educational skills to perform a thorough nutritional assessment and education plan to assist in management of patients with chronic kidney disease, hypertension, and nephrolithiasis. Act as a leader and an integral part of the chronic kidney disease education team, providing support, developing curriculum and protocols specific to the adult chronic kidney disease populations, performing nutrition and educational assessments and providing patient education and family instruction. In addition, have facility with database/data management and standard nutritional software. Participate in the research mission of the Chronic Kidney Disease Program through recruitment of patients for research studies in chronic kidney disease, assisting with consenting patients for research protocols and preparing standardized meals.
