REPORTING RELATIONSHIPS

Reports to Unit Director and/or Unit Manager. Responsible for supervising, directly and through subordinates: Allied Health Professionals, Charge Nurses, Clinical Nurses and non-exempt technical and support personnel.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

1. Consistently supports and maintains the Mission, Core Values and expected customer service behaviors of St. Joseph Medical Center.

2. Follows the St. Joseph Medical Center Guidelines related to the Health Insurance Portability and Accountability Act (HIPAA), designed to prevent or detect unauthorized disclosure of Protected Health Information (PHI)

3. Assists Director/Manager with personnel in assigned unit to ensure provision of effective, efficient and safe nursing care in accordance with established standards, using nursing process to provide all levels of patient care.

4. Renders professional nursing care including administering medications and starting intravenous fluids and blood products. Performs both complex and routine treatments as prescribed by physicians or as warranted by patients condition. Institutes appropriate nursing interventions that might be required to stabilize patience s condition, and/or prevent complications, and to achieve progress toward expected patient outcomes.

* Oversees nursing staff in planning, implementing and evaluating the delivery of patient care according to identified patient needs. Utilizes knowledge and skills of unit and department resource personnel in planning and implementing care, and resolving complex patient care problems. Assists Medical staff in communicating issues and coordinating patient care.

* Communicates with other Hospital departments, as necessary, in order to resolve operational problems and make most effective use of Hospital resources in delivering quality patient care.

* Participates in interdisciplinary patient care conferences necessary in order to plan and evaluate effective patient care.

* Participates in the interview process, coordinates orientation and training, evaluates the performance of and, when necessary, gives input into a corrective Developmental Plan.

* Participates in ongoing quality improvement activities.

* Implements measures to prevent exposure to infection pathogens and communicable conditions; and to promote a safe and confidential environment for the patient and others.

* Maintains knowledge of current trends and developments in the field by reading appropriate books, journals, other literature, attendance at related seminars, conferences and the like.

* Upholds the Code of Ethics and Corporate Compliance.

* Adheres to and helps to enforce all compliance policies relevant to his/her area.

* Assures timely compliance education as requested by the Compliance Officer and/or through corporate initiatives.

* Sets an example to all staff in their daily activities

* Has a basic understanding of Skilled Nursing and Acute Rehab Regulatory Standards.

* Maintains current knowledge and training in all regulatory aspects of Comprehensive Assessments, Assessment Accuracy and Comprehensive Care Plans to include the development of a system of on-going training pertaining to
the SNF population.

* Collects accurate information from patient charts as well as completes all assessments necessary for MDS submission in an accurate and timely manner.

* Screening of possible candidates to admit in skilled nursing unit.

* Completes insurance authorization for admission to the skilled nursing unit.

* Collaborates effectively with HIM: Coding, Revenue Integrity and Billing to ensure accuracy in charting and documentation of records submitted to CMS.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

WORK EXPERIENCE:

* Work requires experience in which managerial and teaching abilities been demonstrated, in order to effectively, manage patient care, unit operations and staff assigned.

* Knowledge/skills in specialty area required in order providing advanced clinical oversight to patient care activities.

* 1 year work experience in a Skilled Nursing Facility and/or Rehab

LICENSE/REGISTRATION/CERTIFICATION:

* Current RN permit or licensure issued by the Board of Nurse Examiners of the State of Texas to practice professional nursing.

EDUCATION & TRAINING:

* Work requires a general level of knowledge and clinical competencies as typically acquired through graduation from an accredited school of nursing program resulting in an Associate Degree, Diploma or Baccalaureate degree in nursing.

* Certification in MDS 3.0 (optional)

* Workshops and continuing education as needed for Skilled Nursing & Acute Rehab

SKILLS:

* Demonstrates proficient interpersonal relationships with employees and medical staff in order to manage professional and support staff, interface with physicians and resolve complex patient care issues.

* Demonstrates technical, clerical, critical thinking skills relevant to total patient care/area in order to safely and efficiently treat patients.

* Basic computer knowledge.

* Able to communicate effectively in English, both verbally and in writing.

* Ability to prioritize work with minimal supervision.
