JOB DESCRIPTION

Directs the day to day supervision of the home health visit staff which includes nurses, home health aides, therapists and social workers. This supervision includes: direct patient care, quality assurance, training and professional education.nnUpholds the philosophy, core values and goals of the organization and ensures the Christian Mission of Lutheran Senior Services is realized so that Older Adults Living Life to the Fullest .nnResponsibilitiesnSupervises home health clinical staff, which includes nurses, home health aides, therapists and social workers.nAssigns clinical staff to new and existing patients.nNotifies nurses and therapists for timely Start of Care/Resumption of Care.nReviews physician and wound care orders.nReviews start of care orders (i.e. 485 Plan of Care).nAssists other QI nurse(s) with auditing of open and closed charts to meet established goals.nRemains available at all times during hours of operation. May be reachable by pager or cell phone. nServes as the on-call person when needed.nParticipates in all activities relevant to the professional services provided.nCoordinates and/or provides for monthly staff in-services.nAssists with the implementation and monitoring of the annual quality improvement plan.nParticipates in the QI committee.nProvides on-site in home evaluations of nurses and home health aides.nAttends staff meetings and reports to Director of Home Health and/or Administrator as often as necessary.nCommunicates with other In-Home Services staff to coordinate services.nPrepares written reports as required by management.nHires, disciplines and fires employees in accordance with LSS policies and procedures.nTrains and orients staff to agency policies and procedures.nEvaluates employees in a direct reporting relationship within three (3) months after hire and annually thereafter.nProvides other supervisory functions as directed, and acts as the Director in his/her absence.nMaintains liaison with community groups, professional organizations and other agencies.nPrepares reports and statistical records as required.nAssures that all clinical documentation is appropriate for billing and reimbursement criteria.nRemains current on all home health regulations (conditions of participation, PPS, OASIS, state regulations, etc.)nnnQualifications, Knowledge, Skills & AbilitiesnnAt least two years management experience in acute care or home health.nCurrent Missouri and Illinois nursing license in good standing.nAt least two years home health experience.nKnowledge of current home health regulationsnExcellent communication skills.nAbility to provide own transportation.

QUALIFICATIONS, KNOWLEDGE, SKILLS & ABILITIES

* At least two years management experience in acute care or home health.

* Current Missouri and Illinois nursing license in good standing.

* At least two years home health experience.

* Knowledge of current home health regulations

* Excellent communication skills.

* Ability to provide own transportation.
