Clinical Team Leader The Evercare Clinical Team Leader is responsible for managing approximately 2535% case load and direct supervisory responsibility for 4 to 6 Evercare MidLevel practitioners. The Supervisor, Nurse Practitioner reports to the Manager, Nurse Practitioner. Responsibilities: * Providing direct primary care to aging residents in longterm care facilities in collaboration with primary care physicians * Coordinating care by collaborating with primary care physicians, consulting physicians, facility staff and patients' families * Participating in care conferences with other providers and patient family members * Educating nursing home staff, physicians and administration. * The Clinical Team Lead will also serve as the direct supervisor for a team of 2 6 Nurse Practitioners * Provides primary case management of chronic and acute conditions.
