Clinical Team Leader The Evercare Clinical Team Leader is responsible for managing approximately 25 35% case load and direct supervisory responsibility for 4 to 6 Evercare Mid Level practitioners. The Supervisor, Nurse Practitioner reports to the Manager, Nurse Practitioner. Responsibilities:Providing direct primary care to aging residents in long term care facilities in collaboration with primary care physicians Coordinating care by collaborating with primary care physicians, consulting physicians, facility staff and patients' families Participating in care conferences with other providers and patient family members Educating nursing home staff, physicians and administration. The Clinical Team Lead will also serve as the direct supervisor for a team of 2 6 Nurse Practitioners Provides primary case management of chronic and acute conditions. Reviews the work of others. Develops innovative approaches. Sought out as expert. Serves as a leader/ mentor. Coordinates, supervises and is accountable for the daily activities of business support, technical or production team or unit. Impact of work is most often at the team level.
