Department Description

The Health information/Medical Record Department is a support service of UCSF Medical Center whose primary purpose is to contribute to the quality of patient care through the development and maintenance of a comprehensive, centralized medical record system.

We currently maintain five million records and support the mission of the Medical Center by providing complete and accurate medical information for patient care, research and financial planning.

Job Summary

The Health Information Coder is a middle level coder.

Patient records are coded and abstracted according to UCSF Medical Center policies and procedures. The focus is on range of hospital charts, with minimal review of coding for quality.

The coding and abstracting must comply with the official guidelines for coding and reporting, practice standards and code of ethics for HIMS coder.

The flexibility to orient and work at all UCSF Medical Center locations is required.

* Assumes accountability for improving quality metrics associated with department/unit and meeting organizational/departmental targets.

Work Environment

* Keeps working areas neat, orderly and clutter-free, including the hallways. Adheres to cleaning processes and puts things back where they belong. Removes and reports broken equipment and furniture.

* Picks up and disposes of any litter found throughout entire facility.

* Posts flyers and posters in designated areas only; does not post on walls, doors or windows.

* Knows where the Environment of Care Manual is kept in department; corrects or reports unsafe conditions to the appropriate departments.

* Protects the physical environment and equipment from damage and theft.

Required Qualifications

* 2+ years experience coding and abstracting in an acute health care facility

* CCS , RHIT or RHIA certification

* High School diploma or equiavalent

* Must code all records with 95% accuracy

* Basic computer skills, including Windows, Word and Excel

* Proven excellent communication and cooperative skills

* Ability to analyze information, make decisions and exercise independent judgement

* Ability to follow written directions

* Ability to prioritize work and manage time effectively

* Ability to meet deadlines and perform well under pressure

* The flexibility to orient and work at all UCSF Medical Center locations is required

Preferred Qualifications

* Knowledge of medical terminology, medical abbreviations, disease processes, anatomy and physiology

* Knowledge of the ICD-9-CM, ICD-10-CM, ICD-10-PCS and CPT classification systems

* Knowledge of HIMS database systems

* Knowledge of HIMS procedures
