Job Summary

Under general supervision and according to established policies and procedures, reviews medical records of moderate to high complexity levels for all types of inpatient and outpatient cases in order to identify relevant diagnostic and procedural information.Assigns diagnostic and procedural codes in accordance with approved coding schemes and established coding guidelines.Enters coded information and, if applicable, assigns DRG and/or APG into computerized database.Abstracts information from medical records of all patient types and enters abstracted information into computerized database.Assists with review of clinical information as requested.Assembles and organizes medical records according to established chart order format.Functions as a coding resource for internal and external customers. Acts as a functional resource for entry level coders and assists in training, as needed.Minimum Requirements

1. Registered Health Information Technician (RHIT) or;

Registered Health Information Administrator (RHIA) or eligible, or;

Certified Coding Specialist (CCS) or; equivalent combination of education and/or hospital coding experience.

2. One to two years technical coding experience which includes experience coding all types of inpatient and ambulatory cases.
3. Experience in computerized encoding and abstracting in a Windows-based environment preferred.
