JOB DESCRIPTIONJOB SUMMARY
Community Health Workers (CHW) is now a more complex position which is now nationally recognized. In addition to acting as a client advocate and liaison, the CHWs now play a significant role in research by recruiting specific clients for research projects and working with them throughout the study. CHWs also provide care coordination to assist patients in reaching a more healthy lifestyle while managing their chronic disease states. This position has one-on-one interaction with patients in order to provide services to at-risk clients and families.
The CHW assesses patients needs for health coverage, self-management health education, makes referral recommendations and assists in accessing services as appropriate. The CHW advocates on behalf of the patients and or families to health and human service providers and agencies.
The CHWs complete home visits as needed. This position will work in what we have determined to be Region 4 and includes Madison, Estill and Powell counties. The home office will be located in one of these counties.
SKILLS / KNOWLEDGE / ABILITIES
Communication skills, computer skills, knowledge of county and state resources.
DOES THIS POSITION HAVE SUPERVISORY RESPONSIBILITIES*
One or more years experience enrolling participants into a health plan or any kind of research projects preferred.
