RESPONSIBILITIES AND QUALIFICATIONS:
It assists welfare recipients with a substance abuse problem to achieve self-sufficiency through treatment and employment. The goal is to help clients comply with their required substance abuse treatment; with other mandates of the welfare to work initiative; and to obtain employment or disability benefits.
Provides direct case management services in collaboration with case management team consistent with VNSNY Home Care policy and requirements of the Comprehensive Care Management Program. Works under general supervision.
Licensure: Valid driver's license may be required, as determined by operational/regional needs.
Education: Bachelor&rsquot;s Degree in Social Work, Sociology, Psychology or other field related to social work required.
Experience: Minimum of one year experience in a human services area required. Experience in case management or field (outreach) preferred. Knowledge of substance abuse/recovery issues preferred. Experience with educational/vocational rehabilitation counseling and solid clinical intake assessment skills preferred. Bilingual English/Spanish preferred.
