Job Description
Performs comprehensive nutrition assessments of assigned patients, develops care plans, and provides medical nutrition therapy, clinical guidance, nutrition education and counseling to meet normal and therapeutic needs in accordance with VNSNY Home Care procedures and regulatory requirements. Works under general supervision.
