Heres your opportunity to get in on the ground floor of an exciting initiative that is transforming the delivery of care. We are currently in the midst of implementing a new Medical Home Initiative (MHI) that will transform our affiliated primary care practices into fully functional Patient-Centered Medical Homes. This initiative, which incorporates best practices is aimed at improving performance on the key dimensions of quality, cost and patient satisfaction, and to prepare for the emerging healthcare environment. As Director of Care Management, you will serve a dual role as: 1) lead care manager (CM) and faculty member for the MHI; and 2) lead practice-based CM for one of the general medicine groups participating in the MHI. You will specifically be responsible for organizing and coordinating the practice teams efforts to support patients most at risk for health deterioration andor poor outcomes; and for training other practice staff to support care management activities. As lead care manager, you will also work with other MHI faculty to design and test processes, procedures, clinical tools and EMR documentation tools used by office practices; monitor and improve performanceresults against specific goals; and provide suggestions for improvement to the MHI and office practice. As the lead practice-based CM you will help to identify and prioritize target patients; and provide patient care outreach, coordination, planning, assessment and self-management. Requires a Registered Nurse (Masters degree preferred); and at least 5 years of clinical and management or system development experience, preferably within the ICU; along with discharge planningcase management, disease management, and system development experience. Position also requires excellent skills in: clinical assessment, problem prioritization and care planning; communication, problem-solving and management; and use of EMR, population registries and other databases
