Director of Case Management
Position Summary: This position supports the mission of The Medical Center of Aurora/Centennial Medical Plaza of optimizing the quality of life of all those served by expanding beyond the traditional focus on medical needs to the needs of the whole person. The Director of Case Management is responsible and accountable for planning, directing, leading, and budgeting for high quality, cost effective services provided by the department to its customers. This requires an understanding of and support of the organization s mission, vision, values, priorities, and financial goals.

A. Licensure/Certification/Registration: Either Social Work or R.N. license required. Certified Case Manager preferred.
B. Education: Bachelors degree required, Masters preferred in either field.
C. Experience: A minimum of five years clinical experience in acute care setting preferred.
D. Special Qualifications: Must have knowledge of JCAHO & CMS standards as well as federal, state, and local laws pertaining to the functions of the department. Must have the ability to assess information, apply critical thinking, determine best course of action, and convey necessary information to the interdisciplinary team. Requires excellent communication skills, both written and verbal. Must be able to prioritize workload and adjust staffing up or down, depending on census and patient need.
Degree of supervision required: Involves general guidance and direction by the Director of Case Management or designee. Employee will be expected to perform most job duties independently and in accordance with established departmental and hospital policies and procedures.
Populations and Ages of Patients Served: This position requires knowledge of growth and developmental needs of patients and families. Personnel in this position must recognize and respond to the behaviors of patients and families in each age category. The employee must be able to demonstrate the special skills required to care for patients and their families according to standards. (The skills and knowledge needed to provide such care may be gained through education, training or experience.)
