Department: Hospital Administration
The Director of Continuing Care, provides operational management and strategic direction for Nursing and Specialty Profession staff in the Office of Care Coordination. Reports directly to the Chief Medical Director of the Health System and is responsible for driving and directing the care coordination system. Establishes standardized policies and procedures for inpatient and outpatient case management and care coordination. Responsible for development, implementation and ongoing administration of care coordination policies. Works closely with the Medical Director and other care providers to develop, implement and maintain policies, procedures and practices that support effective case management, timely access, appropriate utilization management, treatment planning and care transition planning across the care continuum. Assists in facilitating achievement of quality and cost effective patient outcomes and improved throughput. Demonstrates commitment to a patient and family-centered model of care. Actively participates in Utilization Review processes and serves as a quality improvement team member,
Qualifications: Bachelors in nursing and a Masters in nursing or related field required. Minimum of three (3) years of management experience. Experience in care coordination, utilization review, insurance authorization, regulatory compliance, and quality monitoring. Experience working with physicians and healthcare professionals and program development. Experience with Performance Improvement strongly preferred. Relevant experience with children and youth with special health care needs.
License, Registration or Certification required: Current RN License in the state of California required
Knowledge Skills and Abilities Required: Knowledge of healthcare reimbursement mechanisms. Knowledge of current issues and trends in healthcare. Familiarity with principles of managed care and outcomes assessment. Demonstrated strong leadership, problem solving, and data analysis skills. Ability to write clear and concise reports. Well-developed communication and negotiation skills.
