External Care Coordinator is accountable for initiating, maintaining and developing ongoing professional contacts with area healthcare facilities,multidiplinary team and the community. Performs pre-discharge review of all clinical and insurance information. Obtains accurate clinical documentation and written referral.
position will be held at JSUMC.
[Minimum education]
Associate degree of nursing.
[Desired education]
BSN preferred
[Minimum work experience]
Two years healthcare experience preferred. Must possess knowledge of insurance reimbursement.
[Required licenses/certifications]
Current NJ RN License.
[Required skills]
Organized, good communication and computer skills
[Valid drivers license required]
