RESPONSIBILITIES AND QUALIFICATIONS:
It services Medicaid eligible members with a serious mental illness and/or two of the following: substance abuse, HIV/AIDS and/or multiple chronic illnesses.
The position is part of a 1 year partner project with CCB with possible extension after the 1 year period.
Flexible hours to possibly include evenings and weekends.
Assesses patients, develops a care plan, and coordinates care for patients with chronic medical conditions, patient education, patient advocacy, and regular evaluation of patients on caseload. Also evaluates patients&rsquot; eligibility for Health Home At-Risk, Health Home and other social service programs. Alerts members of the care team when follow-up is required and ensures efficient and successful access and linkage to the full array of physical and behavioral health services. Works under general direction.
Licensure: Medical Assistant, LPN, or Patient Care Technician preferred. Valid driver s license may be required, as determined by operational/regional needs.
Education: High School Diploma equivalent required. Associate s or Bachelor s degree in a human services or related field preferred.
Experience:Minimum of one year experience in care management, community health, social service, or medical practice required. Effective oral/written/interpersonal communication skills required. Basic computer skills required. Bilingual skills may be required as determined by operational needs.
