JOB DUTIES:

*

The Health Educator is responsible for the assessment, planning, coordination and implementation of health education program(s) that focus on underserved racial/ethnic groups who are at high risk for illness, disability and death from chronic diseases. Coordinate and implement patient group health education sessions and other health education presentations.
*

Prepare class materials and equipment prior to the classes. Conduct all assessments necessary for the classes such as BMI, pre/post surveys, and weekly goals.
*

Directly provide health education services to Clinic patients in the areas of chronic disease prevention and management, including nutrition, diabetes and asthma management, and hypertension. Assist patients in setting self-management goals during their medical visits.
*

Oversee and manage the health education resource wall and provider charting area to ensure the health education materials are well stocked and readily available for use with patients.
*

Assist with the development, organization and supervision of health education programs and materials for patients that seek to improve the quality of life, reduce morbidity and mortality among patients with asthma, diabetes and hypertension.
*

Develop and manage program tracking systems for budget management and productivity.
*

Oversee and manage health education program needs such as ordering materials, purchasing snacks/food for classes, recruiting/coordinating volunteers, tracking visits/attendance, pre/post data and securing space to conduct the health education sessions/classes.
*

Work closely and communicate effectively with clinical and non-clinical staff to inform them about health education services available and to be informed of other clinic services available for the patients.
*

Gear health education programs that promote efficient resource utilization and ensure optimal outcomes in concert with the patient s physician.
*

Provide input in the program development of wellness programs and interventions.
*

Conduct presentations at community-based organizations when needed.
*

Other duties as assigned.
*

Build and enhance a mutual and interactive relationship with residents of our service area communities, and various community-based organizations, including schools, day laborer centers, and other organizations.
*

Provide information about the Clinic to community members and serve as a liaison between the Clinic and the community.
*

Identify new and maintain existing relationships with community organizations where outreach efforts can be conducted and health education workshops can be offered for the community at large.
*

Develop and distribute outreach flyers.
*

Conduct outreach and provide educational presentations at day laborer sites, street corners and community-based organizations on HIV/STDs, family planning, nutrition, and chronic disease management.
*

Attend outreach events as assigned by the health education supervisor or as needed.
*

Document numbers of individuals reached through outreach activities.
*

Other duties as assigned. Monitor and evaluate targeted intervention practices, contract requirements, and submit complete patient encounter forms in a timely fashion.
*

Conduct reminder and follow-up phone calls to participants of our health education programs.
*

Coordinate and manage health education services tracking and submit monthly reports to the Health Education Services Supervisor.
*

Attend and actively participate in staff meetings.
*

Act as a liaison between community organizations and the clinic.
*

Refer patients to other clinic services and community agencies, as needed.
*

Other duties as assigned.





JOB QUALIFICATIONS:

*

Master s degree in Public Health, Nursing, Nutrition or related field OR a Bachelor s degree in Health or a related field and at two years of experience providing health education services in a public health or primary health care organization.
*

Working knowledge of health education issues and programs including nutrition, lifestyle and behavioral issues.
*

Working knowledge of chronic diseases and strategies to improve patient adherence to medical treatment plans.
*

Bilingual English/Spanish skills.
*

Ability to establish and maintain cooperative working relationships with physicians, clinic staff, patients and volunteers.
*

Ability to work effectively as a member of an interdisciplinary team.
*

Knowledge of the Latino culture and diet.
*

Excellent communication and organizational skills.
*

Ability to effectively communicate with a wide variety of people of various educational and socio-economic backgrounds.
*

Ability to function efficiently and independently, and to plan and prioritize daily work activities to meet clinic goals and objectives.
*

Ability to interact with staff to promote interdepartmental communication.
*

Demonstrated proficiency in using Microsoft applications such as Word, Excel and Access.
*

Ability to travel between clinics and community organizations to conduct health education programs, presentation, services and outreach.
