Facility: Hospice - Meadville at Meadville
Department: Hospice - Home Care Services

Position provides personal care and assists patient in activities of daily living. May perform related housekeeping services for patient as necessary. MINIMUM QUALIFICATIONS: Completed Home Health Aide training and competency program so as to meet the requirments of Medicare. CPR certification required. Credentialing/Competencies in accordance with current credentialing and/or competency requirements. Licensed, or access to transportation, driver with current Pennsylvania License and careful driving record. Auto insurance is required by the State of Pennsylvania and proof of current coverage is required at all times. Act 33/34 Clearance required. PHYSICAL DEMANDS: Position requires ability to assist with patient positioning, transfer techniques and patient ambulation. Work setting may be either that of an office or in the home involving the ability to bend, stoop, lift, twist and maneuver as necessary to conduct patient evaluations, and to access the patients home. Good physical and mental health combined with emotional and mental maturity is necessary to effectively support and meet the patients needs. Ability to hear and communicate in the English language (written and oral), as relative to support and instruction of patients, their families, and in recording of necessary documentation in charting process. Position requires traveling both day and night therefore, requires ability to drive or access to transportation providing staff with ability to provide these services. WORKING CONDITIONS: Possible exposure to various chemicals relating to job performance would be in the use of equipment and I all clinical/medical procedures which may be performed.
