GENERAL DESCRIPTION:
Christiana Care VNA is seeking a part time social worker to work out of our Camden office providing visits to patients in their homes.

PRIMARY FUNCTION:

The Medical Social Worker is responsible to the Care Management supervisor and works in close cooperation with the nursing staff and Care Management Supervisor for the implementation and delivery of Medical Social Work Services.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Essential Job Functions:
Assist the physician and other members of the health care team in understanding the significant social and emotional facts related to patient s health problems.
Make home visits to assess the social and emotional factors in order to estimate the patient s capacity and potential to cope with problems of daily living.
Help the patient and patient s family to understand, accept and follow medical recommendations and provide services planned to restore the client to optimum social and health adjustments within his/her disability.
Assist patient and their families with personal and environmental difficulties which predispose toward illness or interfere with obtaining maximum benefits from medical care.
Utilize resources, such as family and community agencies, to assist patient to resume life in the community or to learn to live within his/her disability.
Act as consultant to agency.
Record recommendations and patient s progress in the agency s clinical record following each client visit.
Provides clinical supervision to Social Work Assistant as assigned.
Marginal Job Functions:
Participate in CCVNA inservice education programs.
Other duties as assigned
Participate in Case Conferences.
Performs assigned work safely, adhering to established departmental safety rules and practices; reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.
Performs other related duties as required.

SCOPE, PURPOSE, AND FREQUENCY OF CONTACTS:

Frequent home contacts with clients/families.
Frequent phone consultation/collaboration with other professionals and community resources.

DIRECTION/SUPERVISION OF OTHERS:

Serves as resource to agency staff.

DIRECTION/SUPERVISION RECEIVED:

Directed by designated supervisor.

EDUCATION AND EXPERIENCE REQUIREMENTS:

Master s Degree from a school of social work accredited by the council on Social Work Education.
One year experience in health care setting.
An equivalent combination of education and experience may be substituted.

KNOWLEDGE, SKILL, AND ABILITY REQUIREMENTS:

Knowledge of community resources.
Expertise in target populations of elderly, disabled, and/or high risk OB patients.

SPECIAL REQUIREMENTS:

Bilingual desirable. Sensitivity to various cultures and values. Proof of a valid driver s license and active auto insurance is required. Alternative transportation arrangements may be considered based on operational need.

PHYSICAL DEMANDS:

Ability to travel to multiple patients daily.

WORKING CONDITIONS:

Works in diverse home environments in varying conditions.
