Qualifications:
Registered Nurse or LPN
Minimum two years home care field experience.
Minimum one year Marketing experience
Above Average computer skills.
Advanced understanding of DRG/ICD9 coding, preferred.
Excellent interpersonal skills.

Responsibilities:

Act as a liaison to hospitals and nursing homes; meeting with discharge planners and social workers, as needed.
Meet with hospitalized clients to discuss SMHH home care program, as appropriate.
Assess referrals and obtain necessary information for appropriate and possible admissions to the homecare program in response to physician s orders.
Coordinates efforts with the billing department in order to verify insurance coverage, client benefits, and visit authorization.
Prepare intake paperwork for charts, including coding the diagnosis and procedures.
Refer clients to appropriate Nursing Supervisor for assignment to appropriate staff.
