HHA/ C.N.A

Functions as a para-professional member of the Hospice team supervised by a Patient Care Manager. Perform various services for a patient as necessary to meet the patient's personal needs, to promote the patient's comfort, and a safe environment. Observe patients and report/document these observations and the services provided. Manage related expenditures in a fiscally responsible manner in accordance with the Company's budget. Travel as necessary.
Requirements

* Minimum age requirement of 18.
* Completion of recognized Nurse Assistant certification program
* Passing score on Odyssey's skills test.
* Minimum of one-year direct patient care experience in an institutional or home care setting required. Hospice experience preferred.
* Skilled in the proper technique for lifting and transferring patients.
* Ability to read, write and follow instructions and document care given.
* Understands Hospice philosophy and needs of terminally ill.
* Compliance with accepted professional standards and practices.
* Demonstrates tact, patience and good personal hygiene.
* Excellent observation and communication skills.
* Ability to work within an interdisciplinary setting.
* Physical exam in accordance with state law requirements.
* Valid driver's license with maintenance of a good driving record and reliable transportation that is insured in accordance with state and/or Odyssey requirements and is in good working order.
* Satisfactory references from employers and/or professional peers.
* Satisfactory criminal background check.
* Self-directed with the ability to work with little supervision.
* Flexible and cooperative in fulfilling all obligations.
